ALL players who wish to participate in SBA Boy's Soccer Spring 2012, must have a Physical Evaluation
form on file with our Athletic Department BEFORE being allowed to take part in conditioning, try-outs,
practices, games, etc.  The forms are available in the SBA school office or you may download them by
